As we approach the end of our fiscal year, we want to take a moment to step back and share what this year has actually looked like, not just in data, but in real moments, real people, and real change.

Gianna & Yasmine: A Permanent Home
Have you ever stopped to reflect on what permanency really means for someone? Permanency comes to life through stories like Gianna’s who went above and beyond to ensure that Yasmine, the girl who would soon become her daughter, grow up in a place where she could find strength, belonging, and every opportunity to succeed.
A New Beginning
In the fall of 2023, Bridges Homeward’s Family Services team began working with Gianna, a 25-year-old school teacher from the South Shore. Gianna volunteered with siblings in foster care, but she realized she wanted to do more; she wanted to adopt and welcome a child permanently into her family. Following her approval in early 2024, Gianna was matched with an 11-year-old girl, Yasmine. When Yasmine needed to move from her long-term foster home, Gianna worked quickly and welcomed her into her home in June 2024.
A Stronger Future
Working closely with Bridges Homeward adoption team and other providers, Gianna developed thoughtful and compassionate parenting strategies. She arranged for Yasmine to spend time with her brother on a regular basis. She helped reconnect Yasmine with her birth mother, whom she had not spoken with in years. On National Adoption Day on November 21, 2025, Bridges Homeward was thrilled to celebrate with Gianna and Yasmine when their adoption was finalized.

Sofia: Finding Community
Becoming a mother is one of the most profound experiences a person can go through. It brings joy, wonder, and an overwhelming amount of questions. For Sofia, it also brought something she hadn't expected: a deep sense of isolation. Young and pregnant, she was navigating one of life's biggest moments without a roadmap or a community to lean on.
That changed when she was connected to Bridges Homeward.
Sofia is a young woman who is expecting her first baby, and like any first-time mom, she has hopes, questions, and a whole lot of love already waiting for her child. At a tender age, the road ahead carries its own unique challenges, but Sofia is not facing them alone. Through Bridges Homeward's Healthy Families Massachusetts program, Sofia was connected with a home visiting specialist who walks alongside her consistently, not just to check boxes, but to listen, encourage, and meet her exactly where she is. HFM works with families who have or are expecting a child under one year old, helping parents build confidence, access resources, and create strong foundations so their families can thrive.
From early on, Sofia discovered something she hadn't anticipated: a community that genuinely cared. Other mothers in the program, young parents navigating the same mix of excitement and uncertainty, welcomed her in from the start. They even donated lightly used clothing their families had outgrown to help her prepare for her baby's arrival. Through Bridges Homeward's partnership with MIT Federal Credit Union, Sofia also attended financial literacy classes alongside other young parents. She walked away with real tools, a clearer vision of her future, and a growing sense that she was capable of building something solid for herself and her child.
The program also introduced Sofia to the importance of early childhood literacy, encouraging parents to begin reading to their babies from the very beginning. It is a simple habit with a profound impact, one that Sofia embraces wholeheartedly because that is the kind of mom she is already becoming.
What Healthy Families Massachusetts gave Sofia, more than anything else, is community. A group of people who get it, who show up for each other, and who make space for her exactly as she is. She came in looking for support and found so much more than she expected.
